Preparation of an additional extract from the medical record

Issuance of a single document Form 027/о (form of primary accounting documentation No. 027/о “Extract from the medical record of an outpatient (inpatient) patient”), approved by the Ministry of Health of Ukraine (Order of the Ministry of Health of Ukraine No. 110 dated 14.02.2012).

The document contains a brief list of doctors' conclusions and examinations that were conducted at the Dobrobut Medical Network.

Duration: 60 minutes
