Extirpation of the bladder for bladder cancer (cystectomy)

Bladder cancer is an aggressive, widespread disease in Ukraine. The main quality treatment for first-stage bladder cancer is the removal of the bladder (cystectomy).

Contraindications for the extirpation of the bladder

Extirpation is contraindicated in case of intolerance to anesthesia and severe concomitant diseases of the heart, liver, kidneys, lungs, blood, diabetes mellitus, exhaustion, systemic infections. Another contraindication is obstruction of the urinary tract, which makes it impossible to create a new pathway for urine excretion from the body.

The principle of the operation

There are 2 techniques for performing this surgery:

open (laparotomic);

endoscopic (laparoscopic).

During laparotomy, a non-media longitudinal or arcuate incision is made in the patient's lower abdomen, which gives access to the bladder. Next, the surgeon crosses the ligaments of the bladder and ligates the vessels that feed the organ with blood. After that, the bladder is removed, followed by the formation of a new pathway for urine diversion.

The course of the laparoscopic operation is similar, but it is carried out through several incisions in the soft tissues, which are inserted by an endoscope - a miniature video camera using a light device - and microsurgical instruments. In terms of effectiveness, such an operation is in no way inferior to an open one, a short recovery period and minimal risk of postoperative complications make laparoscopic surgery the method of choice.
