Stomach cancer is a malignant tumor that occurs from the epithelium of the gastric mucosa. More than 85% of the stomach cancer patients are in the age group over 40 years old, and men are two times more often than women.

Causes and risk factors:

The main reason for the development of stomach cancer is stillnot apparent , but there are clear ideas about the risk factors and predisposing factors.
  • hereditary predisposition;
  • helicobacter pylori infection;
  • polyps of the stomach or adenomatous polyposis;

  • chronic atrophic/hypertrophic gastritis, gastric ulcer
  • pernicious anemia (a chronic disease with the inability to absorb vitamin B-12);
  • previously undergone gastric surgery;


  • smoking;
  • alcohol abuse;
  • systematic consumption of spicy, coarse, scorching food;
  • a high content of nitrates and herbicides in water.

Clinical manifestations:

The initial manifestations of stomach cancer are nonspecific, which complicates the early diagnosis of the disease.

As a rule, these are symptoms of dyspepsia (pain in the upper abdomen, nausea, vomiting, heartburn, belching), which gives reason to suspect a peptic ulcer.

Later, a feeling of early satiety (fullness after eating a small amount of food) joins.

Dysphagia (difficulty swallowing) occurs in the cancer of the cardia when it is difficult to get food from the esophagus.

Weight loss and general weakness are common, usually due to restrictions food intake.

Massive bleeding during vomiting and melena appear pretty rarely. Typically the development of anemia is due to hidden blood loss.

In some cases, the first symptoms are associated with a metastatic process (including jaundice, fluid accumulation in the abdominal cavity, and bone fractures).


Diagnostics:

The first stage in diagnosing cancer of the gastrointestinal tract, including gastric tumors, isan x-ray. An X-ray examination method allows studying the location, size, contour, tumor volume, and evacuation function of the stomach.

The "gold standard" diagnosis of stomach cancer is an upper GI endoscopy or esophagogastroduodenoscopy (EGD). This method of diagnosis makes it possible to detect the tumor at an early stage of development and at the same time to collect material from the mucous membrane (tissue biopsy) for a morphological examination of the tumor. The efficiency of the EGD diagnosis reaches 95-97%.

Instrumental methods are also used at the stomach cancer diagnosis: magnetic resonance imaging (MRI) and computed tomography (CT) to determine the prevalence of metastasis and the preparation of treatment plans.


Treatment:

In the absence of metastases, the main method of treating patients with stomach cancer in Ukraine is a radical surgical operation - subtotal gastrectomy or gastrectomy with the removal of regional lymph nodes.

After surgery, a combination of chemotherapy and radiation therapy significantly improves treatment outcomes.

In the presence of metastases or multiple lesions of regional lymph nodes, radical surgery is not indicated. As a rule, palliative interventions are performed, which involvethe imposition of a gastrointestinal anastomosis in case of obstruction of the gatekeeper,

Another treatment for patients with specific gene mutations is targeted therapy. The essence of this treatment method is the effect of drugs and substances precisely on the mechanisms of tumor cells that promote its growth and spread.
