Pancreatic cancer is a rather rare aggressive cancer with the worst prognosis. Surgical treatment to remove cancerous and precancerous conditions of the gland significantly improves survival. Nowadays, in Ukraine, there are very few clinics that perform pancreatic resection, since this surgical intervention requires high qualifications and skill from a surgeon.
Pancreatic resection is in most cases the only known cancer control method.
Currently, two main methods of resection are firmly established in oncological practice:
The majority of malignant tumors (up to 60%) of the pancreas develop in its head - during this "Whipple operation" is performed; if a tumor is detected in the tail of the pancreas, distal resection is performed, which allows preserving the head of the gland and significant parts of its tissue (which reduces functional damage).

This operation is performed on most patients with exocrine pancreatic cancer, namely head cancer. During the operation, the head of the pancreas is removed (if the body of the gland is also affected by cancer, it is also removed), the duodenum, the gallbladder, and part of the common bile duct connecting it to the duodenum. The remaining element of the small intestine is sutured to the resected part of the pancreas, the stump of the bile duct is connected to the intestine, this is done to restore the normal process of digestion.
There is also the concept of an extended Whipple operation, which also includes the removal of a part of the stomach, as well as lymph node dissection - the removal of a package of lymph nodes located next to the head of the pancreas.
This surgical technique is used to remove the affected distal part of the gland, namely the body and tail. Very often, the spleen is removed at the same time. Such operations are usually performed for neuroendocrine tumors, such as insulinomas. A feature of these tumors is rapid metastasis, which makes surgical treatment impossible.
