Treatment of granuloma: clinical recommendations

Granuloma is a small limited nodular accumulation of granulations in tissues, which develops in response to a chronic inflammatory process or the presence of a foreign body. The causes of granuloma formation are various infectious and non-infectious agents. In some cases, the etiology of the disease is unknown. Postoperative granuloma is a thickening on the surface or in the thickness of the scar. Most often, its formation is a reaction to a foreign body (suture material that does not dissolve). With a purulent granuloma, there is pain, an increase in body temperature, and hyperemia at the site of compaction.

Eosinophilic granuloma

This term denotes a pathology of unknown etiology, which is characterized by intensive proliferation of reticulohistiocytic elements with the formation of infiltrates in the bones.

Bones are usually affected:

  • pelvis;
  • skull;
  • spine.

The disease develops slowly. The first symptoms of eosinophilic granuloma are swelling and pain in the lesion. In the future, spontaneous fractures are possible.

In children, there are cases of spontaneous stabilization and regression of the process (up to full recovery). If necessary, granuloma treatment is prescribed with glucocorticosteroids (local or parenteral), and PUVA therapy is carried out.

Disease pyogenic granuloma

Pyogenic granulomas are vascular nodules of red color with a wet or crusted surface. The size of formations usually does not exceed 20 mm in diameter. The epidermis that covers the vascular nodules is quite thin, so the "sore" bleeds easily. During pregnancy, formations sometimes grow (the so-called tumor of the gums of pregnant women, or telangiectatic epulis).

The disease pyogenic granuloma is diagnosed as follows: tissue excision is performed followed by histological examination. Treatment consists in removal (excision or curettage) and electrocoagulation. Relapses of the disease are possible. Read more about pathology on our website.

How to cure granuloma in a child

The causes of ring-shaped granulomas in children are unknown. Scientists think that cellular immune reactions of the fourth type and pathology of tissue monocytes take part in the formation of the disease. Sometimes clinical manifestations of the disease are observed after prolonged insolation, insect bites, injuries, vaccination.

The development of annular granuloma is a benign process characterized by the formation of nodules or papules that form a kind of ring around areas of unchanged skin. Eruptions of yellowish-brown, flesh-colored or bluish color are located in the form of single or multiple cells. The characteristic localization is the back surface of the hands and feet. Lesions can coalesce, forming rings.

Spontaneous regression of rashes is often observed in children. If the process is widespread or the rash is disturbing (itching, pain) - treatment is prescribed. It is possible to cure granuloma in a child with the help of ointments with corticosteroids or intralesional administration of hormonal drugs. PUVA therapy and pulsed laser are also successfully used.

Granuloma on the tooth root

Periapical and apical granulomas are those that develop on or around the apex of the tooth root. They consist of a proliferative mass of granulation tissue inhabited by bacteria. The formation of "dental" granulomas is an attempt by the body to stop the spread of the infectious process from the teeth to other organs and tissues.

Periapical granulomas grow slowly. They can reach several centimeters in diameter. Small formations (several millimeters in diameter) do not have clearly defined boundaries, so they are difficult to identify. Usually, periapical granulomas do not present themselves with any symptoms. Only a secondary infection that has joined can provoke the development of pain.

Granuloma of the apex of the tooth root is a manifestation of apical periodontitis with the formation of granulation tissue. The main factor in the formation of a granuloma on the root of a tooth is a bacterial infection. A capsule of collagen fibers forms around the granuloma. When a tooth is removed, the sac-like structure is usually pulled out together with the tooth.

Removal of granuloma on the root of the tooth is carried out by means of apical resection. Unfortunately, her only alternative is tooth extraction.
